The position emphasis will be on care coordination, communication and collaboration with nursing, physicians, acute care service provides and insurers to pace the care towards optimal outcomes within appropriate level of care.
The Care Coordinator RN is responsible for assessing, planning, facilitating and advocating for options and services through a continuum of care from point of contact through discharge on assigned patients.
In this role, you will be responsible to supervises and coordinates patient care and field employees for services provided by trinitycare extended care. Works independently in carry out assigned duties in the field with limited supervision.
In this role, the selected candidate will work collaboratively with an assigned panel of physicians to manage the patient's specialized needs; identify member needs and development of specific care management plan to address needs.
In this role, you will manage individualized, goal directed nursing care in accordance with departmental and hospital policies and procedures. He / She will develop and implement plans of care and ensures quality assurance.

The Intake Case Manager screens potential case management patients, completes assessments to determine unmet needs, develops individual care plans, evaluates and identifies knowledge gaps of disease process and treatments, determines appropriate resources or services required to meet an individual's health needs, promotes quality cost effective outcomes with the goal of improved care coordination amongst providers and increased involvement of the individual/family/caregiver in the decision making process to reduce hospitalizations, readmissions and ER visits.
The incumbent will be responsible for the day-to-day management of the Medi-Cal Complex Case Management Program, including overview of targeted members, management of the nurses and staff in the unit, interface with local medical centers and physicians, and monitoring and evaluation of program outcomes achieved.
