The Care Manager is a self motivated health care professional capable of performing complex discharge planning for patients with serious illnesses of different ages and developmental levels focusing on those patients with complexities related to clinical, finance and/or resource utilization components.
The National Eye Care Specialist will lead, sell and promote one or all Eye Care product lines North America Field Sales; in addition, will have personal sales responsibility for Eye Care sales in assigned National Accounts.
The Clinical Care Coordinator (CCC), or Case Manager under general supervision, the CCC acts independently in coordinating and tracking patient care, and providing discharge planning for patients within an episode of illness.

The successful candidate has proven leadership skills and knowledge of state regulatory requirements and URAC. The RN Case Manager will work Monday through Friday during normal business hours in a comfortable and modern professional office environment.
