In this role, you will be responsible for collaboration with other agencies, services, community providers as needed towards achieving discharge; notification and involvement of administrative personnel as needed to achieve a safe discharge.
The Social Worker provides crisis intervention and ensures continuity of care between CPEP and inpatient or outpatient providers, resource consultation, and routine discharge planning for patients they evaluate.
The social worker is responsible for providing clinical social work services to patients admitted to his/her designated surgical units (4-3400) and their families, including adult patients admitted to the various surgical services, and patients of all ages admitted to the unit under other clinical services.
In this role, you will provide hospital patients with psychosocial and continued care planning intervention, serve as a community liaison and provide education in compliance with applicable hospital, departmental and governmental regulations and policies.
The Social Worker works in a collaborative practice and consultation models with various other TW FHT primary health care providers. The Social Worker will also be integral to organizational and community planning, including building partnerships with community resources/agencies, program development, and inter professional education and social work education.
