The Financial Case Manager determines eligibility for Medicaid and other government health insurances, community-based and fiscal programs for patient and families. Develops patient needs-based plan, coordinates services, participates in discharge planning and follows through to completion.
Identify and interact with families and individuals to assess their requirements. Ffind internal and external social service programs that will assist them, assess their most urgent needs, appraise the situation and listen to the clients concerns.
In this role, the selected candidate will participate in client meetings as part of the team representing Manulife Financial; deliver web based presentations to National Rehab team on the topic of Effective Communication.
