Assess the psychosocial status of patients and families / caregivers related to the patients terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group.
Acts as resident/family advocate and ensure the resident is knowledgeable in and exercises his/her rights. Responsible for organizing and conduction of family counsel meetings and maintaining minutes of the meeting.
In this role, the selected candidate will be responsible to performs case finding functions and outreach, identifying and screening patients for psychosocial needs. Independently implements treatment modalities.
In this role, you will be responsible to collaborate directly with the care manager and interfaces with the patient, and/or family and/or the patients representative to assess the patients post-hospital care needs and ensure the patients transition to the least restrictive care environment.
In this role, you will provide psychosocial services to patients treated by the facility including in-center and home dialysis patients (if applicable) utilizing Social Work Theory of Human Behavior and accepted methods of social work practice.
