In this role, the selected candidate will maintain contact with patients, physicians, clinical manager(s), other members of the healthcare team in a timely manner regarding patient/family needs and status changes. Participate in care coordination activities and discharge planning as appropriate.
In this role, you will be responsible for obtaining and documenting in the electronic medical record detailed medical history of each patient, including chief complaint, present condition, individual and family history.
