In this role, you will be responsible for
the tracking and managing of members identified as Intensive Case Management (ICM) who may be high risk, high utilizing, at significant clinical risk or under-utilizing services.
In this role, the selected candidate will coordinate care by serving as the contact point, advocate and resource for the patient, their family and their physician, building effective relationships through trust, respect and communication.
In this role, the selected candidate will demonstrate mastery of patient education in practical applications of a complex nature; possess sufficient knowledge, training, and experience to be capable of successfully delivering patient and family education services without requiring support and instruction from others.
In this role, you will collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individuals benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes.
