The Home Health Team Leader is responsible for the overall supervision and coordination of clinical services. Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high quality care to home health patients assigned to the team's area in accordance with MD prescribed plan of care, and all applicable state and federal laws and regulations.
In this role, you will work under the supervision of the Unit Manager following the American Nurses Association (ANA) standards of practice, and code of ethics within a Patient and Family Centered Care Model.
In this role, you will provide and coordinate patient care, educate patients and the public about various health conditions, and provide advice and emotional support to patients and their family members.
In this role, you will be responsible for network participation, care with specialty networks, care with DME providers and transfers to alternative levels of care using your knowledge of benefit plan design .
This position is responsible for provision of individualized care for patients of all ages. Utilizes delegation and critical thinking skills to provide quality service through a team oriented, collaborative approach to patient care.
In this role, you will manage care/services provided and the development/implementation of patient plans of care for a group of assigned patients within scope of practice, to include performing on-site clinical assessments as appropriate, consulting with third party representatives regarding patient financial responsibility, providing case management and follow up, to ensure care plan stated outcomes are achieved.
