In this role, you will prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, updates/summarizes patient records, and confers with other health care disciplines in providing optimum patient care.
The Psychiatric Nurse Practitioner provides medication management and directs the program of the Assertive Community Treatment Team, a self contained clinical team which assumes responsibility for directly providing needed treatment, rehabilitation and support services to identified clients with severe and persistent mental health illnesses.
In this role, you will provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function or teach compensation measures.
