The SBHC Registered Nurse has the responsibility and accountability for assessing patient/family health problems and resources, taking a leadership role in the planning, implementation and evaluation of the nursing plan of care provided by the nursing team.

In this role, you will prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, updates/summarizes patient records, and confers with other health care disciplines in providing optimum patient care.

This nurses are responsible for providing IV infusion nursing care in the home or ambulatory setting, performing in-hospital discharge assessments and occasional IV hook-ups, teaching patients about their home infusion therapy, and providing ongoing monitoring of their treatment.
