Developing the IFSP (Individual Family Service Plan) in coordination with the IFSP Team. This entails developing Objectives and Goals specific to childs individual needs in one or more of the specified delayed areas.
Perform re-evaluations according to department guidelines, adjusting the treatment plan, adheres to department policy regarding time frame, form and content of written evaluations, progress notes and discharge summaries.
You will put Patient Service first: ensures that patients and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights.
Develops appropriate treatment goals and methods in collaboration with the patient and caregivers, implements the physical therapy treatment plan, and completes all related documentation and record keeping regarding these services.
The successful incumbent demonstrate essential skill competencies in the use of treatment modalities, equipment and protocols.
