Performs psychiatric assessment, formulates diagnosis and in consultation with referring physician recommends and completes appropriate disposition. Discusses case with Attending Physician to determine if the patient has been cleared medically and to determine why a consult is requested. Responds in timely manner to request from Emergency Department Physician for psychiatric consultation.
Interact with patients and families in individual and group settings to perpetually advance the healthy recovery of the patient, and to educate the family throughout the treatment process. Incorporate goals into the treatment plan, and brainstorm with the patient on how to achieve those goals.
Provides clinical social work services or psychosocial assessments to patients/residents and families, regarding the following common problems: adjustment to instituitional living, financial concerns, family relationships, difficulty understanding / accepting medical / diagnoses / procedures / treatments, changes relating to aging, powerlessness, discharge planning and counseling related to death and dying.
The Social Worker is responsible for psychosocial assessments, individual group and family therapy, participation in treatment team planning, processing commitments, discharge planning/case management service, crisis intervention and liaison services with referral sources and other departments within the hospital.
The successful candidate will be responsible for assessing the psychosocial needs of patient/family, crisis intervention, short-term counseling, referral to appropriate rehab and community programs and agencies. Both groups of patients are high utilizers.
Provides psychosocial services to patients treated by the facility including in-center and home dialysis patients (if applicable) utilizing Social Work Theory of Human Behavior and accepted methods of social work practice.
