The Advanced Practice Nurse, Palliative Care promotes high quality, cost-effective palliative care for patients served in a highly regulated and fiscally challenging managed care environment and works collaboration with FRHS interdisciplinary palliative care team, whose aim is to relieve suffering and support the best possible quality of life for patients with advanced chronic or life threatening illnesses, to proactively identify opportunities to improve the patient and family experience of care and improve the efficiency and effectiveness of resource use.
Performs and / or participates in rounds of hospitalized patients, including physical assessment, interpretation of diagnostic studies, interaction with nursing staff and documentation in medical record.
