The unit-based Care Coordinator functions as part of an interdisciplinary team of physicians, nurses and other professionals to maintain continuity of patient care for all patient populations from admission to discharge and utilizes appropriate criteria for inpatient care and timely discharges.
The Member Care Support Network Coordinator will create and maintain partnerships with faith-based organizations in an effort to enhance the ability of the faith community to support the health needs their members.
This position will be part of a dedicated, embedded case management team integrated within the core operations of the Health Care Center at Christiana Practice team of physicians, nurses, nurse practitioners, and other professional staff. The Case Manager will be responsible for collaborating with patients/families, physicians, staff, and other health care professionals across the continuum to provide care coordination.
Utilize clinical judgment, independent analysis, critical thinking, time management and detailed knowledge of the Care Navigation Program to facilitate the coordination of care for patients assigned to the Care Navigation unit.
Write orders for appropriate laboratory EKG and radiology studies. Order appropriate treatments including IV fluids and medications. Request consultations as indicated by patients clinical presentation and acuity.
