Plans, organizes and manages a patient caseload: applies concepts derived from management and communications theory, group dynamics, counseling methodologies, and clinical nursing judgment to implement a clinical nursing program or programs.
Provide integrated and coordinated case management services for the high risk and high utilizer populations in both inpatient and outpatient psychiatry and addiction medicine settings. Responsible for managing all facets of care and utilization of high risk psychiatric and addiction medicine patients.
Provide direct patient care through use of nursing process, performs established nursing intervention utilizing current clinical knowledge, initiates and participates in patient, family and staff education.
Manages individual member cases by coordinating the provisions of services, assuring the cost-effective utilization of health care resources to meet individual members health care needs and promote positive health outcomes.
