The Case Manager plans, organizes and manages a patient caseload: applies concepts derived from management and communications theory, group dynamics, counseling methodologies, and clinical nursing judgment to implement a clinical nursing program or programs.
It involves telephonic outreach to perform comprehensive assessments of the patients condition, determination of available benefits and resources, and development and implementation of a care management plan with performance goals, monitoring and follow up.
In this role, the selected candidate will be responsible for continuous review of all aspects of every patient on his/her caseload to include: appropriate utilization of services; ensuring continued skilled need; monitoring of homebound status; review of documentation in the medical record; maintenance of interdisciplinary communication and discharge planning.
