In this role, the selected candidate will
coordinate appropriate care, encompassing various healthcare personnel (such as Physical Therapists, Occupational Therapists, Home Health Aides and external providers).
The POC Registered Nurse in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations and agency policies.
The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. Administers skilled nursing care to patients requiring intermittent professional nursing services. Teaches and supervises the family, and other members of the nursing team. Each staff nurse performs nursing services for residents within a specific area.
The RN provides nursing care to patients in compliance with the SC Nurse Practice Act, Standards of Practice and defined by ANA, SFHSC, and organization protocols, policies, procedures and guidelines. Develops,implements, evaluates and revises plan of care based on physical, psychosocial, spiritual, and educational needs of the patient/family.
In this role, you will demonstrates professional growth and development through involvement in professional organizations, reviews professional literature and participates in continuing education programs.
The Registered Nurse provides nursing care to patients in compliance with the South Carolina Nurse Practice Act, Standards of Practice defined by ANA, Joint Commission, and organization protocols, policies, procedures and guidelines.
