The selected candidate will be responsible for performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.
The case manager is responsible to communicate with external services and agencies to expedite the process of selecting/approving/certifying services needed during the patients acute care and transition to a post- hospital setting.
