Provide guidance and direction to subordinate staff and for organizing, directing, coordinating, and controlling activities for the care of patients, the development of personnel, compliance with regulatory and professional standards, strategic planning and the fostering of interdisciplinary collaborative relationships.
Supervises clinical staff and facilitates communication with physicians and internal administrators to provide optimal patient and family-centered care. Is recognized as a change agent and routinely leads efforts to strengthen organizational systems that support the mission, vision, and values of the department.
The Nurse Case Manager collaborates with other members of the case management and health care teams, utilizes professional nursing skills to: assist the patient in achieving optimal health; assure appropriate use of health care resources; facilitate/coordinate timely care along the continuum; decrease unnecessary variances in care through proactive case management; develop a safe and timely discharge plan to meet the patients needs.
The Nurse Manager assumes 24-hour responsibility for planning, supervising, and evaluating the care of patients to ensure comprehensive and continuous care for each patient/family in the Neonatal Intensive Care Unit (NICU).
Responsible for performing care management within the scope of licensure for members with complex and chronic care needs by assessing, developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize member health care across the care continuum.
