The RN UNIT REGISTERED NURSE delivers quality nursing care to patients through interpersonal contact and provides care and services to allow patients to attain or maintain the highest practicable physical, mental, and psychosocial well-being in accordance with all applicable laws, regulations, and Life Care standards.
In this position, you will works with Care Coordination MVP Team members to assess, plan, implement, coordinate, monitor, and evaluate services and outcomes to maximize the health of the Member. The POC Registered Nurse in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations and agency policies.
The RN is responsible for assessing, planning, implementing and evaluating nursing care and provision of safety to assigned patients, and/or patients on the Med/Surg Unit, utilizing the developmental theory of the pediatric, adult and geriatric patient.
Provide professional nursing care to patients utilizing the nursing process. To collaborate with other members of the health care delivery team to deliver quality care in accordance with the philosophy, goals, and objectives of CMC.
