The Registered Nurse (RN) in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations and agency policies.
In this role, you will be responsible to provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer on-going care and case management for each patient, provide necessary follow-up as directed by the clinical manager.
Provide hands-on care, management and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision; revise plan in consultation with physician based on ongoing assessments and as required by policy/regulation.
In this role, you will participate in performance improvement activities, maintain ongoing clinical knowledge through internal/external training programs, provide interpretation of knowledge and direction to staff.
Assists physician and senior department personnel in conducting invasive tests and procedures by preparing equipment and patient for exam, monitoring patient's vital signs and electrocardiograph data, administers medications under physicians supervision and attending to patient's needs under minimal supervision and according to established policies and procedures.
As a Registered Nurse or Licensed Practical Nurse, you will render professional nursing care to patients in their home by assessing, developing, implementing, and evaluating home nursing care needs of assigned patients.
